Nondegree students, former students, students who previously received a UIC College of Applied Health Sciences Scholarship Fund, and students enrolled in other colleges at UIC are not eligible.<\/p>\n<h2>Criteria:<\/h2>\n<p>This award is to recognize outstanding student leaders who maintain high academic achievement, have financial need and demonstrate a commitment to volunteerism within the UIC community and greater Chicagoland area (e.g., through participation in student organizations, campus activities and local nonprofits that strengthen the community).<\/p>\n<h2>Award Amount (up to $1000):<\/h2>\n<p>The college will present up to 5 awards.<\/p>\n<p><strong>Application Form:<\/strong><\/p>\n<p>To apply for this award, you must be a full time student (12 credits for undergraduate or professional levels, 9 credits for graduate level) and submit the following:<\/p>\n<ul>\n<li>Complete <strong>both<\/strong> the online general application and the AHS Scholarship Fund Award application questions through <strong>UIC SnAP by <\/strong><strong>February 7<\/strong>.<\/li>\n<li>Obtain a letter of support from a faculty member or advisor.<\/li>\n<li>Submit personal statement as required in application.<\/li>\n<li><strong>Application<\/strong> form is available online at the link below.<br \/>\n<a href=\"https:\/\/uic.academicworks.com\/users\/sign_in\">https:\/\/uic.academicworks.com\/users\/sign_in<\/a><\/li>\n<\/ul>\n<p><strong>Instructions<\/strong> for completing application through UIC SnAP:<\/p>\n<p>1)\u00a0\u00a0\u00a0 Sign In with Your Institution using your UIC email address and password<\/p>\n<p>2)\u00a0\u00a0\u00a0 Click on \u201cApplications\u201d in menu bar<\/p>\n<p>3)\u00a0\u00a0\u00a0 Click on \u201cGeneral Application\u201d<\/p>\n<p>4)\u00a0\u00a0\u00a0 Complete required fields in the General Application<\/p>\n<p>5)\u00a0\u00a0\u00a0 Upon completion, click on \u201cUpdate your Application\u201d<\/p>\n<p>6)\u00a0\u00a0\u00a0 Next, click on \u201cOpportunities\u201d in menu bar.<\/p>\n<p>7)\u00a0\u00a0\u00a0 Use the pull down menu to be sure that \u201cAll\u201d is highlighted and not just \u201cRecommended\u201d scholarships<\/p>\n<p>8)\u00a0\u00a0\u00a0 Then you can search for \u201cAHS Scholarship Fund Award\u201d<\/p>\n<p>9)\u00a0\u00a0\u00a0 Complete the required fields in the application: 1) personal statement, 2) letter of support, and 3) general question(s)<\/p>\n<p>10)\u00a0 Upon completion, update\/save\/submit your application.<\/p>\n<p>Should you have any questions, please contact the AHS Student Affairs Office at 312-996-2079.<\/p>\n","intro_text_excerpt":"Deadline for the AHS Scholarship Fund award applications is February 7th, 2020.","content_author":"Eileen M.
