From School to Adulthood: Collaborative Strategies for Successful Transitions

Preparing students with disabilities for life after high school requires thoughtful, collaborative transition planning. This session will equip professionals with evidence-based strategies to strengthen family and student engagement in the secondary transition process. Participants will explore practical tools to support person-centered planning, promote student voice and self-advocacy, and build partnerships between schools, families, and adult service systems. The presentation will also address common barriers to engagement and share promising practices that foster meaningful post-school outcomes in employment, education, and community living.
