Enabling adults with chronic conditions to participate in the occupations that are meaningful to them and their families
We evaluate the efficiency and quality of the tasks of everyday living, the impact that habits and routines have on individuals’ health and wellness, and intervene prior to disability and when illness/injury/chronic conditions create barriers to independence and participation in meaningful occupations. OT self-management services promote long-term successful engagement and management of health.

Outcomes of our services will result in an increase in self-efficacy and self-management of disability and chronic conditions; decreased hospitalization and reliance on costly medical services; improved performance of occupations, quality of life and well-being; and increased participation within home, work, and community environments.
Our providers are skilled in providing:
- Occupation-Based Self-Management training
- Motivational Interviewing to promote lifestyle change
- Culturally- Tailored Healthy Lifestyle Interventions
- Evidence-informed, theory-driven practice
- Interventions to address health disparities
- Client-driven action planning and goal setting
- Environmental modification to promote engagement in meaningful occupations
- Translation of health recommendations to the client’s everyday life

UI Health Diabetes and Endocrinology Center
Collaborative interdisciplinary services are primarily provided in the clinic alongside endocrinologists, nurses, dieticians and diabetes educators. When possible, services are provided in the community. In addition, regular phone and email follow-up support is provided as needed to ensure continuity of care and additional self-management support for clients served in the clinic.
UI Health Family Medicine Clinic
Primary care occupational therapy services are provided in the clinic. Home visits are provided when patients are unable to come to the clinic for services. In addition, regular phone and email follow-up support is provided as needed to ensure continuity of care and additional self-management support for clients served in the clinic.
