Official UIC application
Submit your UIC application online via the UIC website—just follow the instructions listed there. There is a non-refundable application fee of $70 for all applicants.

You don’t have to enter and submit all application information in one sitting. You can return to the system as many times as you need to. You can also return to the system checklist as often as you like to see what materials have been uploaded and verified as received.

Transcripts
Submit unofficial copies of official registrar-issued transcripts from institutions where you earned a degree and from all colleges and universities attended. A link to upload these will be in the application provided.

Personal Statement
Required to submit a personal statement that does not exceed two pages. The statement should address three elements: 1) the applicant’s career vision; 2) the applicant’s rationale for pursuing a research doctorate to advance their career vision; and 3) the applicant’s clinical, teaching, and research experience(s) that shaped their interest in pursuing a research doctorate.

Letters of recommendation
Three required letters on official letterhead, from individuals who can explain their relationship to the applicant and assess the candidate’s aptitude and potential to complete a research doctorate.

Curriculum Vitae
Required to submit a current CV, including scholarly activities and record of publications and presentations to date.

Faculty commitment to advise
Because this program follows a mentorship model, it is important that applicants identify and contact at least one desired faculty mentor/advisor working in their desired specialty area prior to applying and secure faculty signature on the form that gets submitted as part of the application.

If English is not your native language, you might need to take the Test of English as a Foreign Language (TOEFL), the exam of the International English Language Testing System (IELTS) or the PTE-Academic. Check out the International Requirements to see if this applies to you.

Scores must be submitted to UIC directly using the institutional code 1851.
